Job Summary:

The Wellness Consultant collaborates with the Wellness team to deliver coordinated Health Promotion events, activities, and dissemination of resource materials. Partner with other Wellness Consultants to develop, coordinate and deliver holistic, culturally-honoring wellness programming and promote Un'ina (customers) wellness outcomes by facilitating effective self-management. Disseminate best practice guidelines to Dena'ina Wellness Center staff via electronic means, uploaded into the Wellness Coaching toolkit, and provide ongoing training to staff as needed. Direct service delivery of Wellness Coaching for individuals, families and groups, as well as staff wellness education. Provide feedback to integrated care team regarding current/developing policies and impact on health of clients, collect and disseminate relevant outcome data, incorporate standards of care, and participate in evaluation process. Closely involved with delivering prevention and promotion aspects of national service frameworks and strategic plans for public health and the development of community strategies and local strategic partnerships. Work to ensure effective practice by building capacity and are committed to promoting health equity.

Essential Functions:

  • Provide Health Promotion for individuals and groups inclusive of integrated care team members via collaborative planning.
  • Participate in the delivery and administrative support of programs, projects, and events that promote traditional, holistic health and wellness improvement for Un'ina (customers) of all ages.
  • Participate in the planning of the wellness class schedule in collaboration with the integrated care team.
  • Provide coordinated Wellness Coaching including comprehensive assessment and appropriate intervention in collaboration with the integrated care team.
  • Research and maintain a library of best practice curricula and resources.
  • Deliver the strategies for promoting health and wellness at the local level.
  • Develop the public health skills and capacities of individuals, groups, and organizations, and empower them to make or support heathy choices.
  • Support multi-agency/department collaborative projects to promote a healthy context or social environment.
  • Participate in continuous quality improvement process and customer satisfaction activities.
  • Provide Un'ina (customers) with information or resources as requested and facilitate warm transfer/referral to appropriate care team member.
  • Effectively utilize the electronic health record to document wellness services.
  • Provide Un'ina (customer)-centered action planning utilizing motivational interviewing principles.
  • Participate in care team training and learning activities.
  • Maintain a personal level of physical, mental and spiritual wellness appropriate to serve as a role model.
  • Lead activities appropriate to certification.
  • Participate in traditional seasonal activities including harvesting, preservation, preparation and presentation of traditional foods.
  • Attend and facilitate staff meetings as required.
  • Participate in work groups as assigned.
  • Perform other duties as assigned by the supervisor.

Working Conditions:

Light work - Exerting up to 20 pounds of force frequently, and/or a negligible amount of force constantly to move objects. If the use of arm and/or leg control requires exertion of forces greater than that of sedentary work and if the worker sits most of the time, the job is considered light work.

Qualifications:

Education:

  • Bachelor's degree in health education, health promotion or related field.

Experience:

  • Minimum of two (2) years of experience in health and wellness program delivery
  • Experience with computerized information management systems
  • Experience working with Alaska Native/American Indian people preferred

License/Certification:

  • Certified Health Education Specialist (CHES) preferred
  • Behavioral Health Aide certification preferred
  • Tobacco Treatment Specialist Certification (TTSC) or eligible for certification
  • State of Alaska Food Worker Card
  • CPR and AED certification
  • Possess a valid driver's license

Special Skills:

  • Able to handle multiple projects simultaneously and to organize, establish priorities, follow-up on all details, and meet deadlines
  • Excellent interpersonal communications, public speaking, and presentation skills
  • Able to provide courteous, collaborative and culturally-honoring service to all Un'ina (Customers), staff and visitors, and must be a team player
