Job Summary

The Panel Management Assistant is responsible for utilizing the Dene' Model of Care to provide integrated support and care coordination for Un'ina and the care team.  The Panel Management Assistant will utilize knowledge and skills with available resources, tools, and data to proactively support comprehensive and preventative wellness of Un'ina. 

Essential Functions

  • Utilizing the Dene' Model of Care and health care management tools, to gather information needed to coordinate health care needs of assigned panel(s).
  • Utilize available tools to support assigned panel(s) with needed data and resources.
  • Update patient care information in existing systems
  • Advocate for comprehensive and collaborative Un'ina wellness needs.
  • Educate and support Un'ina appointments and follow up
  • Communicate care coordination to Un'ina in a manner that empowers them
  • Follow up with Un'ina regarding completion of care needs related to referrals
  • Coordinate care that supports Dene' Model of Care internally
    • Recognize and advocate support for unique communication and social needs of Un'ina
  • Coordinate care that supports Dene' Model of Care with external parties
    • Manage Un'ina Travel as part of the continuity of care process
  • Work collaboratively by building bridges and creating rapport with all team members within the organization, the Alaska Native Tribal Health System, and other referral agencies, functioning as a liaison between specialty providers, and Un'ina.
    • Proactively provide essential support and collaborate with Care Team

Qualifications

Education

  • High School Diploma or general education degree (GED)

Experience

  • One year of medical or general office experience

License/Certification

  • None

Special Skills

  • Good oral and written communication skills
  • Excellent telephone and customer service skills
  • Excellent organizational skills and ability to work in a fast paced work environment
  • Knowledge of Microsoft Office and other applications
  • Knowledge of Electronic Health Records and Systems

Preferred Qualifications and Certifications

  • CPR and First Aide certification
  • Knowledge and experience working with Alaska Native/American Indian people
